This program is for children and youth, 7 -17 years of age, who have not been baptized

  • 7 – 17 years of age and have not been baptized

• Documents needed
  • Original Birth Certificate (no copies)

• Sponsor Documents Needed
  • An updated Baptismal Certificate with notations
    • Notations must include 1st Communion, Confirmation, and Marriage (If applicable)
    • Parents and sponsor(s) MUST take a Pre-Baptismal Class prior to the child receiving the sacraments. (Our Lady of Guadalupe offers Pre-Baptismal Classes. For more information click here)

• Donation
  • $200 donation for the 2 years of formation
